Declaration by parent or guardian

In the event of illness or injury occurring to my son/daughter during his/her attendance at the 6th Baloo Camp, by Ahmadhiyya International School, I hereby consent to advance to whatever medical or surgical diagnostic procedure or treatment is considered necessary in the best judgement of the attending First Aider/doctor and performed by or under the supervision of a member of the medical staff furnishing medical services.

Outing staff shall ensure safety of all students at all times during the camp hours and shall be the utmost priority before carrying out any activity. In case any student is found to be ill, emergency contact will be informed and necessary actions will be taken to manage the condition.

I hereby agree and have no objection in sending the aforementioned participant to 6th Baloo Camp, under the guidance of scout leaders and teachers.

I understand that, in the event of a serious illness or injury, reasonable efforts to reach me will be attempted.
